The Cape Coral Daily Breeze tells MHProNews sales of manufactured homes and homesites have begun early in North Fort Myers, Fla. Margaret Black of Horizon Village Co-op, Inc. says this summer was the busiest she?s ever seen at Horizon, a resident-owned 55+ community of half shareholders and half renters. ?Our annual sales jumped by 30 percent in 2011 and 2012 over the previous years,? she said. Bob Duncan of nearby Windmill Village, noting sales of homes are up at this Equity LifeStyle Properties MHC, says ?We are also bringing in some more new homes next month because of the growth.? Christopher O. Jackson of North Fort Myers Chamber of Commerce says, ?Today?s communities are more upscale with larger homes, lots and amenities. Manufactured homes of years ago were about 12-14 feet wide. Today?s homes can be up to 18 feet wide and up to 80 feet long, a far cry from several decades ago.?

Nonprofit Organizations Fearful of 'Fiscal Cliff' - Philanthropy Today ...

Charities are mobilizing in Washington as the looming ?fiscal cliff? makes politicians on both sides of the aisle more open to limiting tax breaks for wealthy donors, Time writes. The newly formed Charitable Giving Coalition, which includes several of the country?s biggest nonprofit groups, has planned a ?Protect Giving-D.C. Days? lobbying push on December 4 and 5.

The coalition contends President Obama?s proposed cut in the charitable deduction for families making $250,000 or more a year would cut annual giving by $5.6-billion. Charities also oppose competing proposals that would cap overall itemized deductions and let taxpayers choose which breaks to take. Some such plans would ?virtually eliminat[e] the charitable deduction for a certain class of people,? said Steve Taylor, vice president for public policy at the United Way.

Third Way, a Democratic advocacy group, said its proposal for $35,000 cap on itemized deductions that would exclude the charitable tax break has been well-received on Capitol Hill.

Gerson Therapy for Cancer? | Care2 Healthy Living

Gerson Therapy is a largely diet-based alternative treatment for cancer invented by the late Dr. Max Gerson. What have 65 years of medical research concluded about its efficacy and safety?

According to a recent review out of Sloan-Kettering in the journal Oncology, for about $16,000 you can fly to a clinic in Mexico and spend three weeks ?consuming fresh raw fruit and vegetable juices? and ?eliminating salt from the diet.? Sounds pretty benign. Patients are also given a number of supplements including pancreatic enzymes as well as coffee enemas to ?stimulate metabolism.? I do not deny that coffee enemas would be stimulating, but I don?t recommend them given the case reports I note in my 2-min. video Gerson Therapy For Cancer where they?ve killed people. To their credit, modern Gerson practitioners have moved away from the original tenets of the plan, which included feeding people raw calf liver smoothies, after too many people died from systemic blood infections. (These infections were from the same fecal bacteria in chicken I cover in my video?Poultry and Paralysis). After learning of the outbreak, staff at the Gerson Institute decided the policy of drinking blended liver was to be altered and they apparently started injecting raw liver instead.

But hey, conventional cancer treatments are no walk in the park either! The reason people choose them is in hopes that they work. How does the Gerson therapy compare? The first formal investigation into the treatment was back in 1947, and in the 65 years since there?s been over a dozen studies published. Most came to the same conclusion: Gerson therapy is useless or worse.

These negative reviews of Gerson Therapy were written before a head-to-head trial was published of a Gerson-style (Gonzalez) regime versus chemotherapy in terms of survival and quality of life for pancreatic cancer patients. That?s the study I profile in the video above. Hugh Jackman in Talks for X-Men: Days of Future Past

Apparently Charles Xavier (James McAvoy) and Magneto (Michael Fassbender) didn?t listen when Wolverine told them to, ?Go fuck yourself,? because Hugh Jackman is reportedly in negotiations to reprise his role in the sequel, X-Men: Days of Future Past. A brief, but fan-favorite moment in Matthew Vaughn?s X-Men: First Class, the gruff response from Logan when asked to join the X-Men was spot-on for his character and delivered by the face of the franchise in Jackman. With the director of two of the original films, Bryan Singer, now directing the sequel, it?s even more likely that we?ll get to see another classic moment. Hit the jump for more.?

Heat Vision reported that hugh-jackman-the-wolverineJackman was in talks to reprise his role for a cameo in?X-Men: Days of Future Past.?He would join other?X-Men?stars, Ian McKellen and Patrick Stewart, as we reported earlier. The?First Class?sequel not only occurs before the events of the original?X-Men?trilogy, its source material also includes two alternate timelines, which makes it feasible for the casts of both series to appear in the same film. In Chris Claremont?s?Days of Future Past?arc, an alternate future saw surviving mutants held in concentration camps while giant robot Sentinels patrolled the land; ?most of the X-Men had been killed. It fell upon heroes of a prior timeline to stop a key event from happening that would lead to the alternate future. How the two versions of Professor X and Magneto are going to be used in the film remains to be seen.

It?s also unclear at this time just how much screentime Jackman would see. With him wrapping on?Wolverine?(which focuses quite a bit on the hero?s struggle with immortality), he could do press for his upcoming film, Les Miserables, and still be in fighting shape by the time cameras start rolling on the sequel early next year. Whether he?ll have a ten-second cameo or a more integral role in the film, we?ll just have to wait and see.

So, for those of you who are keeping score,?X-Men: Days of Future Past?is set to star Patrick Stewart, Ian McKellen, James McAvoy, Michael Fassbender, Jennifer Lawrence, Nicholas Hoult?and Hugh Jackman. Crazy.

X-Men: Days of Future Past is set to open July 18, 2014.

Former baseball star indicted for insider trading

LOS ANGELES (AP) ? Former Baltimore Orioles all-star Doug DeCinces and three others were indicted Wednesday on insider trading charges involving the use of information prior to the takeover of a medical device company in California, authorities said.

In 2008, DeCinces was told by a close friend and official at Advanced Medical Optics Inc. that Abbott Laboratories planned to pay $21 to $23 a share for the company's stock, prosecutors said. At the time, Advanced Medical Optics was trading for about $8 a share.

DeCinces began buying Advance Medical stock based on that information and passed along the takeover details to three friends because he wanted to make up for previous investment recommendations that had gone bad, prosecutors said.

When Abbott's offer was made public, DeCinces sold his newly purchased shares and profited about $1.3 million, court documents show. Abbott acquired Advanced Medical Optics in January 2009.

DeCinces' three friends each bought and sold Advanced Medical stock and made anywhere from $140,000 to nearly $350,000, authorities said.

Also indicted on insider trading charges were David Parker, 60, of Provo, Utah; Fred Scott Jackson, 65, of Newport Beach, and Roger Wittenbach, 69, of Lutherville-Timonium in Maryland.

DeCinces, 62, was charged with 42 counts of securities fraud and one count of money laundering. Each of the fraud counts carries a maximum sentence of 20 years in prison. DeCinces is scheduled to appear in court on Dec. 17.

"To put it mildly, we do not agree with the decision to bring a criminal case against Doug DeCinces," said Gordon Greenberg, DeCinces' attorney. "But the proper place to have this resolved is in the courtroom and not on the courthouse steps."

Messages left for the other defendants' lawyers were not immediately returned.

The Securities and Exchange Commission previously filed a civil lawsuit against DeCinces that was settled last year when the former major leaguer agreed to pay $2.5 million. In the lawsuit, federal authorities named James Mazzo, Advanced Medical's former CEO, as the person who told DeCinces about the pending deal.

Mazzo is a senior vice president at Abbott and is not facing criminal charges. In August, in response to the SEC lawsuit, Mazzo's attorney said his client would be fully exonerated after a review of the evidence.

Also caught up in the insider trading case was Hall of Famer and DeCinces' former teammate Eddie Murray, who agreed in August to pay nearly $360,000 to settle federal civil charges. Federal investigators said DeCinces told Murray about the impending takeover. Murray neither admitted nor denied wrongdoing.

DeCinces was a third baseman during 15 seasons with the Orioles, California Angels and St. Louis Cardinals. He currently is president and CEO of a real estate development firm in Orange County.

Small Business Saturday boosts some local shops | small, saturday ...

Sales at World of Wigs in Santa Ana soared 500 percent on Small Business Saturday, compared with a typical Saturday. And Toy Boat, Toy Boat, Toy Boat, a Corona del Mar toy store, tripled its sales from the post-Thanksgiving sales event in 2011.

American Express started promoting Small Business Saturday between Black Friday and Cyber Monday in 2010 to encourage shoppers to support their local merchants. This year, the credit card giant offered signs, door mats and $25 gift cards that participating merchants could give away and credit that its cardholders could receive on their next bill by shopping at qualified small businesses.

American Express reported Monday that about 500,000 small businesses nationwide participated this year. The company has not yet said how many consumers shopped locally on Saturday compared with the 103 million who spent money at small businesses during the 2011 Small Business Saturday.

But anecdotally, the day was a success for some Orange County merchants who had special activities and sales on Saturday.

?It was fabulous, one of best Saturdays we've had,? said Susan Upton, owner of Lolo Boutique in Fullerton, who offered a sparkling wine reception Saturday. ?Sales were twice what they were last year. It was fun; the people were nice and excited about what we had to show.?

The combination of Fullerton Chamber of Commerce support and American Express promotions brought more attention to Small Business Saturday, Upton said, adding that the boutique's patrons also filled two baskets with toys for dogs in animal shelters for its ongoing Santa Paws campaign.

Toy Boat owner Lori Curtin said, ?Our Small Business Saturday promotion went great! People from all over Southern California came in.?

A pre-event American Express commercial featured Toy Boat, and on Saturday KNX/1070 AM interviewed Curtin in a news report. ?That was a surprise blessing,? she said Monday.

The shop offered free food and children's arts and crafts projects. ?We also collected a half box of Toys for Tots,? Curtin said. ?We gave customers who contributed to that fundraiser an extra 15 percent discount. It was a heart-warming day.?

Rhonda Thaut of World of Wigs said the store had not had much business on past Small Business Saturdays, so owners decided to reach out to customers to spread information to boost sales. The wig shop offered door prizes, snacks, gifts and hourly flash sales. Customers responded this year as never before.

?People would call in and ask, ?What's on sale now?'? Thaut said. ?It was so surprising. We had half off everything in the store one hour; buy one, get one free another hour. And some of our wigs sell for $3,000, so those were good offers.

?We had so much fun,? she said. ?We had Halloween wigs on clearance, and we even had a couple of Santas come in and buy wigs. Some post-surgery patients said they didn't realize they could refreshen their wigs. They were grateful to get information they couldn't get from the hospital.?

Laguna Beach Books offered half off selected art books on Small Business Saturday, and owner Jane Hanauer said, ?We had a ton of customers, many visitors as well as regular customers. I haven't compared it to a year ago yet.?

The OC Mart Mix, a collection of small, owner-operated businesses, was especially busy, said spokeswoman Allison Olmstead. ?What we found is that Small Business Saturday supporters aren't the early-morning shoppers like Black Friday shoppers, but they are the most thankful and kind group of shoppers out there. They were so supportive and thrilled to be helping the small, owner-operated businesses at the Mix.?

Those entrepreneurs included Sarah Dowdell, owner of Deer Lovely and Whiskey Tango 1977. ?We more than doubled our numbers from last year,? Dowdell said. ?We saw a lot of families and people spend the day with us thanks to the holiday activities, the music, and the photo booth at the OC Mart Mix and the SoCo Farmers' Market.?

Another was Alex Forsythe, owner of Analog. ?Having a day to support small businesses is like a dream come true when you are in your first year of business. So it was great to see so many people come out to support us,? he said.

Jayson Hanson, owner of Stoned Jewelry added, ?It was one of our biggest days this year. It was busy all day. It felt like the economy was back. The Small Business Saturday momentum kept going through Sunday.?

Portola Coffee Lab and Theorem had lines of customers out the door from 7 a.m. to 8 p.m. Saturday, said manager Jeff Clinard. ?We took that time to connect with our customers, answer questions about our brewing methods and even employed a hand-held device so we could ring up customers who also wanted to buy coffee beans and gift cards while standing in line.?

Birth Control Pills: Should They be Available Over-the-counter ...

Many of you may have heard that last week, the American College of Obstetricians and Gynecologists (ACOG), recommended that birth control pills be sold over-the-counter without the need for a health care provider?s exam or prescription. The main reasons behind their recommendation are to increase access and to decrease the cost of birth control pills with the hopes of reducing the number of unintended pregnancies that occur in the U.S.

While we, at Virginia Women?s Center, appreciate the reasons behind this recommendation, we are concerned about the potential repercussions that could arise if birth control pills were available over-the-counter. It?s important to note that an annual exam with an OB-GYN or women?s health nurse practitioner is much more than a birth control prescription or a Pap test.

An annual exam includes a complete review of a woman?s medical history, a pelvic exam, a breast exam, health counseling and the opportunity to ask questions and build a relationship with her health care provider.

Additionally, it may include a discussion on contraception, lab tests or a Pap test. By making birth control pills available without a prescription, many women could miss out on catching health issues or diseases at an earlier time ? when they are easier and less expensive to treat.

Additionally, while birth control pills are for the most part very safe, they are not the right choice in contraception for all women. As Dr. Peter Zedler says, ?There have been many times that patients have come into my office with the plan to use certain types of contraception. However, after we discuss the different options available as well as the risks and benefits of each, they change their minds.? By having birth control pills available over-the-counter, this important discussion with a health care provider may not happen.

One of the reasons why ACOG is recommending this move to over-the-counter birth control pills is because of the cost associated with going to the doctor to obtain a prescription. In the Richmond community, there are several low-cost options for women without insurance. In addition, if birth control pills were sold over-the-counter, they would not be covered by insurance, similar to condoms today. This could actually increase the cost of birth control pills.

At Virginia Women?s Center, we emphasize the importance of preventative care. We see great value in our annual well-woman visits and enjoy partnering together to create an individualized health care plan, which includes a discussion on the appropriate contraception choice for each patient.

How to use deliberate practice to improve your writing ? The Thesis ...

Last week, as one of my last duties as research fellow at RMIT before I take up my new role at ANU, I hosted two seminars ? one from Prof Anthony Pare from McGill and the other from Prof Helen Sword, the writer of the wonderful ?Stylish Academic Writing?.

Helen and Anthony had many interesting observations about the process of learning to write, but both made the same basic point: very few of us have formal instruction in how to write like an academic.

As Anthony Pare pointed out, when you write essays as an undergraduate you are ?eavesdropping? on expert conversation; as a PhD student you are expected to be a part of that conversation. Writing for a teacher is easier than writing for your peers because the expectations are clearer. This is why many PhD students experience what he calls the ?J curve?: a rapid drop in confidence, but a long term upswing as this new kind of writing practice is mastered.

Helen Sword explained that we pick up most of what we know about academic writing informally, from our supervisors and from reading; this learning process is adhoc, idiosyncratic and individualised. Informal learning has benefits, but generally speaking it?s harder to reach expert level unless you really push yourself. Helen noted that many people don?t even consult writing books or take advantage of the free advice on the web and therefore never reach an expert level.

One of the ways to push yourself to become a better writer is to engage in deliberate practice, a term I encountered in Cal Newport?s recent book ?So good they can?t ignore you? (an excellent read by the way). Deliberate practice is a way of consistently stretching yourself out of your comfort zone and thereby increasing your skill.

I thought I?d expand on some of the concepts of deliberate practice via my own recent experience with exercise. Like many academics I?m not very body conscious, but I do try to be active because I know I?m built to survive famines and easily put on weight. In the past I have been into swimming and lifting weights at the gym; solo exercise sessions where I can be alone with my pain.

The problem with solo exercise routines, as you probably know, is you are accountable to no one but yourself. Recognising we had a mutual problem in this area, my friend Joyce (@catspajamasnz) and I became exercise buddies.

The first thing Joyce suggested we try was a Zumba Fitness class. I?ll admit, I was not super enthusiastic. All I knew about Zumba was dance videos full of impossibly swelte people, wearing 80?s outfits and?fluorescent shoes, swinging their dreadlocks around to Latin dance music. I told Joyce I didn?t like Latin dance music, mostly because of the accordions, but I went along with it.

Colour me surprised when I discovered that I LOVE Zumba. I don?t want you to get the impression I love Zumba because I am good at it, because I?m really not. I am so bad at it it?s laughable.

I laugh when I watch myself do awkward white girl dancing in the mirror and I laugh at the insanely silly accordian music. Laughing helps me stick with it because earning Zumba is not easy. According to the wikipedia entry, Zumba involves a mix of

?hip-hop, soca, samba, salsa, merengue, mambo, martial arts, and some Bollywood and belly dance moves?

In other words, it?s complicated. In case you?ve been under a rock for the last decade, this video gives you an idea of what it looks like:

The hardest part of Zumba, for me, is how unfamiliar the movements feel: I?ve danced before, but not like this. Writing like an academic used to unfamiliar too; both like and unlike any writing I had done before. Anthony Pare shared a quote from a PhD student who compared ?academese? to a foreign language. This is a nice description because part of the PhD process is learning to adopt a new kind of scholarly voice.

It takes time to be comfortable speaking ?academese? because it?s profoundly unnatural. The longer you persist at academic writing however, the more natural it will feel, so don?t wait until you are writing chapters or journal papers to start ?speaking like an academic?. Practice this new academic voice as you take notes.

One way to do this is by paying close attention to the verbs. For example, if you were taking notes from this blog post, instead of writing:

?Learning Zumba is an example of deliberate practice in action?

You could write:

?Inger Mewburn argues that learning Zumba is an example of deliberate practice in action?

Including the verb ?argues? implies what Kamler and Thomson would call a ?hands on hips? stance. By pointing out that I am making an argument, as opposed to, for example, stating a fact, you are taking a position on what I am saying, rather than just copying it down.

If you were to take a deliberate practice approach, you would look to how you could extend this ?positioning? of yourself. For example, you could rewrite the note and include some of your own thoughts, like this:

?Inger (2012) claims that learning to do Zumba is an example of deliberate practice in action. While Zumba is difficult to master, and it may take time to get the basics down right, what Inger fails to realise is that learning Zumba is not at all like learning to write. For one thing, you need a computer, not fluorescent shoes.?

That?s pretty good ? can you see what I just did? I just wrote a bit of a thesis in that note. I took a position on what Inger was saying about Zumba and I pointed out the tenuous, even ridiculous, connection between Zumba and writing.

In academic writing, how you say what you say is as important as what you say. Verbs are judgmental. By shifting the verb ?argues? (a neutral kind of verb) to the more aggressive ?claims? (a verb which implies I have not used enough evidence) I created what you might call a ?meta text?: signalling to the reader what I think about Inger?s statements about Zumba, without directly saying it.

The verbs I used in the above example lent my writing a negative critical stance. Deliberate practice involves repeating the same activity over and over, but striving to be better each time. So you could try adjusting your stance to see if you can improve on it. For example, I could try again, keeping my note critical, but using my verbs in a kinder way, starting with a positive verb like this:

?Inger (2012) explains how Zumba can be thought of as an example of deliberate practice in action. Her?comparison between writing and Zumba is tenuous, but her main point is clear: both are complicated and take a long time to learn. In addition,?fluorescent shoes look great ? on writers as well as Zumba lovers.?

Shifting backwards and forwards between positive and negative stances in your notes is like working on Zumba step technique: it helps you to move your thinking around and make it more flexible.

Using your notes as a place to deliberately practice academic writing has two main benefits. It?s ?low stakes? writing; since you are the only audience for your notes, you can try out different points of view and play with ideas. Secondly, taking notes with verbs helps you to form ?mini chunks? of text that can be cut and pasted into your thesis, saving you time.

Look for tools to support your deliberate practice. For example, I keep a list of verbs on my wall so that I make sure to use a wide variety of them in my writing and thereby produce more sophisticated metatext. I make a point of regularly looking up the precise meaning of verbs I use, even familiar ones,?in order to keep my writing precise.

Deliberate practice can increase enjoyment along with skill. A basic tenet of fundraising is people do not give unless they are asked. While this is a good rule if you are a charity raising funds, it can be a source of frustration for individual donors. Often friends and clients tell me they don?t feel in control of their charitable giving because they are always reacting to requests. Here is a simple way to become more proactive in your giving.

1. Identify an issue that is important to you. Choose an issue that reflects a life experience, a media story, or something that has affected a friend, neighbour or colleague. In other words, start with the cause, not the charity.

2. Go to the new free database Charity Focus. On the left hand side choose ?Advanced Search?. In the search bar ?All of these words? type in your issue or a related key word. For example, if you are interested in ?shelters for women and families? type in the key words: women?s shelter, or just shelter. I also recommend that you include a location, such as a province or municipality. Like any database, you will need to refine the search to get the best list.

3. The Advanced Search function will turn up a variety of charities to do further research on. Look at the activity descriptions. Review the financial statements. Go to their website. Do a Google search to cross reference. Get a feel for the culture, philosophy and reach of the charity. While the numbers are important, part of being a proactive is finding organizations that are effective, but may have limited resources.

4. Narrow your list to two or three charities. If you want to take it to the next level, call them and say you are interested in finding out more about their work. Even a brief interaction on the phone can tell you volumes about their culture.

5. If you like what you hear ? or read after Step 3 ? make a donation. (You can do so by CanadaHelps.org on the Charity Focus website.) The gift doesn?t have to be large, but it helps if it is personally significant. Request the charity to stay in touch by email so you can find out more about them over the next year.

Charity Focus makes it easier than ever to be a proactive philanthropist. It requires a bit a legwork, but you get a much deeper knowledge of the charities you support when you initiate the relationship.

Arafat's grave opened for poison tests

RAMALLAH, West Bank (Reuters) - Forensic experts took samples from Yasser Arafat's buried corpse in the West Bank on Tuesday, trying to determine if he was murdered by Israeli agents using the hard-to-trace radioactive poison, Polonium.

Palestinians witnessed the funeral of their hero and longtime leader eight years ago, but conspiracy theories surrounding his death have never been laid to rest.

Many are convinced their icon was the victim of a cowardly assassination, and may stay convinced whatever the outcome of this autopsy. But some in the city of Ramallah where he lies deplored the exhumation.

"This is wrong. After all this time, today they suddenly want to find out the truth?" said construction worker Ahmad Yousef, 31, who stopped to watch the disinterment, carried out behind a wall of blue plastic near the Palestinian presidency headquarters.

"They should have done it eight years ago," he said.

French magistrates in August opened a murder inquiry into Arafat's death in Paris in 2004 after a Swiss institute said it had discovered high levels of polonium on clothing of his which was supplied by his widow, Suha, for a television documentary.

"Samples will be taken according to a very strict protocol and these samples will be analyzed," said Darcy Christen, spokesman for Lausanne University Hospital in Switzerland that carried out the original tests on Arafat's clothes.

"In order to do these analyses, to check, cross-check and double cross-check, it will take several months and I don't think we'll have anything tangible available before March or April next year," he added.

Arafat was always a freedom fighter to Palestinians but a terrorist to Israelis first, and a partner for peace only later. He led the bid for a Palestinian state through years of war and peacemaking, then died in a French hospital aged 75 after a short, mysterious illness.

No autopsy was carried out at the time, at the request of Suha, and French doctors who treated him said they were unable to determine the cause of death.

But allegations of foul play immediately surfaced, and many Palestinians pointed the finger at Israel, which confined Arafat to his West Bank headquarters in Ramallah for the final two and a half years of his life after a Palestinian uprising erupted.

Israel denies murdering him. Its leader at the time, Ariel Sharon, now lies in a coma from which he is expected never to awake. Israel invited the Palestinian leadership to release all Arafat's medical records, which were never made public following his death and still have not been opened.

FRENCH INVESTIGATORS

Polonium, apparently ingested with food, was found to have caused the death of former Russian spy Alexander Litvinenko in London in 2006. But some experts have questioned whether Arafat could have died in this way, pointing to a brief recovery during his illness that they said was not consistent with radioactive poisoning. They also noted he did not lose all his hair.

Eight years is considered the limit to detect any traces of the fast-decaying polonium and Lausanne hospital questioned in August if it would be worth seeking any samples, if access to Arafat's body was delayed as late as "October or November."

Not all of Arafat's family agreed to the exhumation, and his wife Suha chose not to attend the operation she had prompted.

Working in parallel with the forensic team, French magistrates were in Ramallah this week to ask if members of Arafat's inner circle might be able to shed light on his death.

One source told Reuters the French had a list of 60 questions, and had questioned one man for five hours.

Many Palestinians acknowledge that a Palestinian would almost certainly have had to administer any poison, wittingly or unwittingly.

Breast Cancer Screenings

In an opinion piece in the November 21 edition of the?New York Times, H. Gilbert Welch, M.D., M.P.H., writes that regular mammography screenings to detect breast cancer are ?not all they?re cracked up to be.? The appearance of the column coincided with publication of an?article in the?New England Journal of Medicine?(NEJM) co-authored by Welch and oncologist Archie Bleyer, M.D.

Dr. Welch is a professor of Medicine at the Dartmouth Institute for Health Policy and Clinical Research, and a regular op-ed contributor to the?Times. His numerous submissions have included columns such as ?Overdiagnosis as a Flaw in Health Care? and ?What?s Making Us Sick Is an Epidemic of Diagnoses,? which contend that Americans are undergoing costly, unnecessary tests and procedures that are doing more harm than good.

His latest column, titled ?Cancer Survivor or Victim of Overdiagnosis?,? argues that American women are being victimized by overzealous breast cancer screenings, leading to overdiagnoses and harmful, unnecessary treatments?all with a ?limited impact on breast cancer mortality.?

As noted on the Dartmouth medical school?s web site, ?Dr. Welch?s research has focused on the problems created by medicine?s efforts to detect disease early: physicians test too often, treat too aggressively and tell too many people that they are sick.? In particular, he has taken aim at what he claims is overutilization of cancer screenings?not only for breast cancer, but for melanoma, thyroid, lung and prostate cancer.

The principal theme of Welch?s November 21?Times?column?and the NEJM article?is that breast cancer screenings in the US over the past three decades have led to an overdiagnosis of early stage breast cancer. He asserts that more than a million women who received such diagnoses ?underwent surgery, chemotherapy or radiation?for a ?cancer? that was never going to make them sick.? His conclusion? Women and their health care providers should question the value of screening for these cancers.

Welch notes that the research published in the NEJM article shows that about 1.5 million additional women received a diagnosis of early stage breast cancer as a result of mammography screenings. But he laments that the research ?found that there were only around 0.1 million fewer women with a diagnosis of late-stage breast cancer.?

A table in the NEJM article shows that, according to the data studied by the authors, an estimated 67,000 fewer women have been diagnosed with late-stage breast cancer as a result of screenings. However, the authors do not consider this reduction in late-stage cancer diagnoses to be statistically significant to support routine mammography screenings.

The?Times?column also refers to three investigations in Europe in the last two years that showed ?mammography has either a limited impact on breast cancer mortality (reducing it by less than 10 percent) or none at all.? Translated roughly to US mortality rates, cancelling out a 10 percent reduction in mortality would mean that an additional 4,000 women would die each year from breast cancer if they did not undergo mammography screenings.

In addition, by Welch?s own admission, ?it?s impossible to know which women? who received an early stage diagnosis of breast cancer and underwent treatment might have contracted an advanced stage of the disease if they had not undergone screenings. Apparently, their health and lives are also not statistically significant.

As with other calls for a halt to ?unnecessary? screenings and treatments?for prostate and cervical cancers, for instance?surgery and chemotherapy carried out after a positive diagnosis, and any resulting negative effects, are equated with the screenings themselves. With any diagnosis, patients should consult with their physicians about the appropriate course of treatment. In the opinion of Welch and the other charlatans railing against cancer screenings, however, ignorance is bliss and women would be better off if they had no knowledge of their early stage cancer.

The NEJM article notes that over the past three decades, among women 40 years or older, ?deaths from breast cancer decreased from 71 to 51 deaths per 100,000 women?a 28 percent decrease. This reduction is probably due to some combination of the effects of screening mammography and better treatment.?

Although the authors write that ?screening mammography might be responsible for as little as 28 percent or as much as 65 percent of the observed reduction in mortality,? they assert that screenings? contribution to this reduction is small, and that most of it is a result of better treatments. Welch writes in the?Times, ?Our therapies for breast cancer are simply better than they were 30 years ago.?

Welch maintains rather that the overdiagnosed victims of breast cancer screenings are being inflicted with ?some pretty serious harm,? although neither he nor the NEJM article specifically explains the disabilities, suffering and mortality rates associated with what Welch refers to as ?pre-emptive mammography.? (As an aside, isn?t ?pre-emptive? diagnosis of a potentially fatal disease precisely the point?)

He goes on to disparage women whose lives may have been saved through regular screenings. ?The truth is,? he writes, ?those survivors are much more likely to have been victims of overdiagnosis.? If Welch admits that it is impossible to tell who might live without receiving this ?unnecessary? early treatment, isn?t the fact that these women are alive an argument for regular screenings?

The appearance of Welch?s op-ed piece in the?Times?and the newspaper?s implied agreement that routine breast cancer screenings cause harm are in line with the?Times??years-long campaign contending that people in the US are receiving too many screenings, treatments and procedures, and that major cuts can be made in health care costs without negatively affecting patient care.

Such cynical arguments formed the basis of the?Times? support for the Obama administration?s health care overhaul, which was signed into law in March 2010. In reality, the Affordable Care Act has nothing in common with providing improved health care for working people.

In the aftermath of the 2012 elections, such crude arguments based on shoddy science?questioning the value of breast cancer screenings and other vital medical tests and procedures?are aimed at supporting even deeper cost-cutting measures for the government and corporations at the expense of the health and lives of ordinary Americans.

112 killed in fire at Bangladesh garment factory

DHAKA, Bangladesh (AP) ? At least 112 people were killed in a fire that raced through a multi-story garment factory just outside of Bangladesh's capital, an official said Sunday.

The blaze broke out at the seven-story factory operated by Tazreen Fashions late Saturday. By Sunday morning, firefighters had recovered 100 bodies, fire department Operations Director Maj. Mohammad Mahbub told The Associated Press.

He said another 12 people who had suffered injuries after jumping from the building to escape the fire later died at hospitals. The death toll could rise as the search for victims was continuing, he said.

Local media reported that up to 124 people were killed in the fire. The cause of the blaze was not immediately clear, and authorities have ordered an investigation.

Bangladesh has some 4,000 garment factories, many without proper safety measures. The country annually earns about $20 billion from exports of garment products, mainly to the United States and Europe.

Relatives of the factory workers were frantically looking for their loved ones. Sabina Yasmine said she saw the body of her daughter-in-law, who died in the fire, but had no trace of her son, who also worked at the factory.

"Oh, Allah, where's my soul? Where's my son?" wailed Yasmine, who works at another factory in the area. "I want the factory owner to be hanged. For him, many have died, many have gone."

Mahbub said firefighters recovered 69 bodies from the second floor of the factory alone. He said most of the victims had been trapped inside the factory, located just outside of Dhaka, with no emergency exits leading outside the building.

Many workers who had taken shelter on the roof of the factory were rescued, but firefighters were unable to save those who were trapped inside, Mahbub said.

He said the fire broke out on the ground floor, which was used as a warehouse, and spread quickly to the upper floors.

"The factory had three staircases, and all of them were down through the ground floor," Mahbub said. "So the workers could not come out when the fire engulfed the building."

"Had there been at least one emergency exit through outside the factory, the casualties would have been much lower," he said.

Many of the victims were burned beyond recognition. The recovered bodies were kept in rows on the premise of a nearby school.

Army soldiers and paramilitary border guards were deployed to help police keep the situation under control as thousands of onlookers and anxious relatives of the factory workers gathered at the scene, Mahbub said. He would not say how many people were still missing.

Prime Minister Sheikh Hasina expressed shock at the loss of so many lives in the blaze and asked authorities to conduct thorough search-and-rescue operations.

The Bangladesh Garment Manufacturers and Exporters Association said it would stand by the victims' families.

Bangladesh's garment factories make clothes for brands including Wal-Mart, JC Penney, H&M, Marks & Spencer, Carrefour and Tesco.

Separately, a flyover under construction fell onto a busy market, leaving at least 14 people dead including three construction workers in southeastern city of Chittagong, an official said Sunday.

Local fire official Abdul Mannan said the concrete structure collapsed on Saturday night, and authorities recovered the bodies by Sunday morning from under the debris in the second-largest city after Dhaka.

Larry Hagman was a character on screen and off

"Dallas" made Larry Hagman a star, and the latest TNT reboot of the classic show brought him back home to the role and the city he loved.

In January, Hagman appeared at the 2012 Television Critics Association's winter press tour to promote "Dallas" 2.0.?There, he was doing what he did best ? telling a good story. This one was about an old woman coming up to him during his original ?Dallas? years, when his character, the villain J.R. Ewing, was one of the most popular personalities on television:

?She was in a walker and she came up and said, ?You rascal, you. Treating Sue Ellen like that!? and she hit me with her handbag and she knocked me off my chair,? said Hagman, who then got an apology from the woman. ? 'Oh, my husband died recently and I have his pearl-handled six shooter in my bag.' Only time I was pistol-whipped.?

The Fort Worth native's eyebrows were a bit bushier and his face a little more lined than in his younger days, but he still had that mischievous twinkle in his eyes as he talked to reporters about reprising his role as the most notorious bad guy on television in the new ?Dallas.?

He was 80 years old then, and he had just announced he was dealing with throat cancer.

The actor died Nov. 23 at age 81 from complications from his battle with the disease. His family -- including wife Maj Axelsson, whom he married in 1954 -- was by his side in Dallas, where he was working on season two of the revival. Axelsson was the one who convinced him to take on the role of J.R. Ewing because, as she told Hagman, "there isn't one redeeming character in the whole script."

?Even though ?(I Dream of) Jeannie? is on everywhere on cable, only the older folks remember me from that,? Hagman said of his character, Maj. Anthony Nelson, at TCA. ?But everyone remembers J.R. Ewing and ?Dallas.? ??

He said people love the nasty guy for a very basic reason:??Everybody has a jerk like this in the family. That?s what makes him so appealing. Everybody knows who he is.?

A character in real life
The actor?s eccentric ways were well known. He once appeared as a guest at the annual TCA awards show wearing a suit that looked like something he wore back in the day when shooting ?Dallas? during its epic 1978-1991 run.

Turned out it was.

And as he chatted with reporters before the ceremony, he talked about grabbing the old suit out of his closet to wear. He then fished out an elaborate pillbox from his pocket.

?Victoria (Principal) gave this to me for Christmas one year,? Hagman said of his ?Dallas? co-star. ?I must have put it in my pocket that night and I forgot what I did with it. Now it?s like I have a new present from her.?

Earlier in his career, Hagman started observing a silent day once a week. He would not speak to anyone for 24 hours.

?I did it because I was doing ?Jeannie? and I would go out and party on the weekend. I was at a rodeo with all the dust and horse s--- blowing around and I got a sore throat,? Hagman told reporters. ?My doctor told me not to talk for the rest of the weekend and I liked it so much, I did it for one day each week for the next 20 years.

?It was a great way to get a lot of attention,? he explained. ?People are brought into your bubble and they have to concentrate on you in order to communicate. I loved it.?

Though the actor was diagnosed with liver cancer in 1995 and later underwent a liver transplant after years of heavy drinking, Hagman never expected to live forever, just to live the life he had.

?I?m never bored. I?m back in the role I loved, doing what I love and with the people I love,? Hagman said of resurrecting J.R. for the ?Dallas? reboot. ?I?m having a great time.?

The actor filmed six of 15 episodes for the second season of TNT's "Dallas" reboot before he passed away. The new season premieres on Jan. 28.

The obesity epidemic did not result from conscious decisions by children to gain weight or by parents to over feed their children or discourage them from being active. Lack of time has made us more reliant on fast or processed food. Sprawling suburban neighborhoods without sidewalks have reduced opportunities for children to walk to school. Concern about neighborhood safety has reduced opportunities for children to play outside. School cafeterias often sell attractive high-calorie foods to make the money they need for some of their expenses.

Although family rules about food and television time are essential, efforts must be made to make schools and communities healthier places for children. Some of these changes can come from active parent-teacher organizations that push schools to serve healthier options in cafeterias and vending machines, restore physical education programs, or stay open so communities can use the gyms after hours. Efforts to require new schools and parks to be located in neighborhoods where children can walk to them will encourage children to be more physically active. Walking to school with your child and other children is a twofer? it gives you and your child the health benefits of physical activity and can be a special time for talking.

Iraq, Afghan war veterans joining Congress

WASHINGTON (AP) ? As Tammy Duckworth sees it, her path to Congress began when she awoke in the fall of 2004 at Walter Reed Army Medical Center. She was missing both of her legs and faced the prospect of losing her right arm.

Months of agonizing therapy lay ahead. As the highest-ranking double amputee in the ward, Maj. Duckworth became the go-to person for soldiers complaining of substandard care and bureaucratic ambivalence.

Soon, she was pleading their cases to federal lawmakers, including her state's two U.S. senators at the time ? Democrats Dick Durbin and Barack Obama of Illinois. Obama arranged for her to testify at congressional hearings. Durbin encouraged her to run for office.

She lost her first election, but six years later gave it another try and now is one of nine veterans of the Iraq and Afghanistan wars who will serve in next year's freshman class in the of House of Representatives.

Duckworth carries the highest profile of the incoming vets. She was co-piloting a Black Hawk helicopter in Iraq when a rocket-propelled grenade landed in her lap, ripping off one leg and crushing the other. At Walter Reed, she worried about what life as a double amputee had in store. But during her recovery, she found a new mission ? taking care of those she describes as her military brothers and sisters. That mission led her to a job as an assistant secretary at the Department of Veterans Affairs during Obama's first term.

"Had I not been in combat, my life would have never taken this path. You take the path that comes in front of you," Duckworth said from a wheelchair last week as she and her fellow freshmen went through orientation at the Capitol. "For me, I try to live every day honoring the men who carried me out of that field because they could have left me behind, and they didn't."

Duckworth is one of two freshmen Democrats who served in Iraq or Afghanistan. The other is Tulsi Gabbard of Hawaii, who served near Baghdad for a year and was a medical operations specialist. Gabbard said she hopes the two of them can be a voice for female veterans and the unique challenges they face.

Cotton said he could easily see himself working with Duckworth and Gabbard on veteran's issues. "They've carried a heavy load and we owe them a great debt," he said.

At the same time, it's clear the freshmen veterans have clear differences of opinion over policy matters. For example, Gabbard is a strong critic of the war in Afghanistan. She says the United States needs to get out as quickly and safely as possible. Cotton opposes setting timetables for withdrawal.

"We're trying to win a counter-insurgency war where we can put a friendly, allied, stable government in place," Cotton said. "It's certainly been a long and somewhat winding road, but on the whole, America and our interests in the world are much better off for having waged the war in Afghanistan."

There also will be differences over spending priorities. Cotton is reluctant to trim spending on defense as a way to deal with the deficit.

Duckworth said certain programs need close examination, particularly in the area of government contracts. She said she "can actually stand up and talk about defense spending in a way that will be realistic without being attacked for lack of patriotism or not being strong on defense."
